Sciatica describes pain travelling along the sciatic nerve, usually into the buttock and down the leg. It is most often caused by irritation of a nerve root in the low back, commonly from a disc bulge, and less often by tightness or compression deeper in the hip.

Most episodes of acute low back pain improve significantly within two to six weeks. Sciatica can take longer, often several weeks to a few months, and recovery is usually faster when movement and a graded exercise plan start early.

Staying gently active is better than bed rest. Short periods of relative rest during a severe flare are fine, but returning to comfortable movement early is linked to faster recovery and fewer recurrences.

Usually not. Imaging often shows changes that are normal for your age and do not match the source of pain. Your physiotherapist screens for the specific signs that warrant imaging and refers you when it is appropriate.

Yes. Most disc herniations improve without surgery. Treatment focuses on settling nerve irritation, restoring movement, and rebuilding strength and tolerance, with surgical referral reserved for cases that do not respond or that show progressive weakness.

Recurrence is common when treatment stops as soon as the pain does. Rebuilding hip and trunk strength, improving lifting mechanics, and gradually increasing your capacity for work and training all reduce the chance of the next episode.
